1. Acceptance of Terms
By accessing or using the HyperBeRx website ("Site") and our hyperbaric oxygen therapy services ("Services"), you agree to be bound by these Terms & Conditions. If you do not agree to these terms, please do not use the Site or Services.
2. Medical Disclaimer
The information provided on this Site is for general informational and educational purposes only. It is not intended to be, and should not be construed as, medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified healthcare provider with any questions regarding a medical condition.
Hyperbaric oxygen therapy (HBOT) is a medical treatment that requires a prescription and physician oversight. Treatment eligibility is determined following a comprehensive medical evaluation by our physicians. Not all patients or conditions qualify for HBOT.
3. Services
HyperBeRx provides physician-led hyperbaric oxygen therapy services. Our services include but are not limited to:
- Medical consultations and evaluations for HBOT eligibility
- Hyperbaric oxygen therapy treatments as prescribed by a physician
- Follow-up care and progress monitoring
- Insurance verification and billing services
All services are subject to medical evaluation and physician approval. We reserve the right to refuse service if a patient is deemed ineligible for treatment based on medical criteria.
4. Appointments and Cancellations
Patients are expected to arrive on time for scheduled appointments. If you need to cancel or reschedule, please provide at least 24 hours' notice. Repeated no-shows or late cancellations may result in scheduling restrictions.
5. Insurance and Payment
HyperBeRx works with a variety of insurance providers. Coverage for HBOT depends on your specific plan and the medical condition being treated. We will assist with insurance verification, but coverage is not guaranteed. Patients are responsible for any copays, deductibles, or out-of-pocket costs as determined by their insurance plan.
For patients without insurance coverage, payment arrangements must be made prior to beginning treatment.
6. Patient Responsibilities
As a patient, you agree to:
- Provide accurate and complete medical history and personal information
- Follow all pre-treatment and post-treatment instructions provided by our medical team
- Disclose all medications, supplements, and medical conditions
- Comply with safety protocols during treatment sessions
- Inform staff immediately of any discomfort or adverse reactions during treatment
7. Informed Consent
Before receiving HBOT, all patients must sign an informed consent form acknowledging that they understand the potential risks, benefits, and alternatives associated with the treatment. This consent is separate from these Terms & Conditions.
